HFDI Supports Integrated SRHR Data Validation at Ojomu PHC, Offa

Hope for Family Development Initiative (HFDI) participated in the monthly data validation meeting held at Ojomu Primary Health Care Centre in Offa Local Government Area.

 

The meeting began featured the local government Monitoring and Evaluation Officer, the Disease Surveillance and Notification Officers (DSNOs), PHC representatives, and facility data focal persons from selected health facilities across Offa LGA.

 

The session focused on strengthening the quality, accuracy, completeness, and timeliness of Sexual and Reproductive Health and Rights (SRHR) data within the LGA health system, implemented by HFDI and sponsored by Jhpiego, a global non-profit health organisation.

 

Participants reviewed routine health service data submitted by facilities and validated reported indicators to ensure consistency between source documents, facility registers, summary forms, and reporting tools.

 

Each facility representative presented registers for the review and validation of both SRHR and communicable disease indicators as part of the integrated monthly process.

 

The Key indicators examined included family planning services, antenatal care attendance, HIV testing services, STI case management, malaria cases and treatment records, immunization data, and other routine health services.

 

Facility reports were cross-checked against registers and monthly summary sheets to identify discrepancies, incomplete entries, and areas requiring correction before final submission to the national reporting platform.

 

The LGA M&E Officer emphasized the importance of accurate documentation, proper use of data collection tools, and timely submission of reports to reduce inconsistencies.

 

Special attention was given to ensuring that figures on summary forms correspond with entries in source registers, particularly for SRHR indicators that are often underreported.

 

Hope for Family Development Initiatives HFDI supported the process by following up to ensure SRHR indicators were adequately captured and reflected in the National Data Repository and routine reporting tools.

 

According to the representative of the organization, the follow-up contributed to improved visibility and inclusion of SRHR data within the integrated disease surveillance validation process at the LGA level.

 

The participants also discussed common reporting challenges, including incomplete documentation and delays in submission. The meeting strengthened collaboration between surveillance officers, facility data focal persons, and PHC representatives, promoting a more integrated approach to health data management in Offa LGA.

 

At the close of the session, participants agreed on the need to continue integrating SRHR indicators into monthly validation meetings to sustain improvements in data quality, reporting accuracy, and evidence-based health planning.
